Junior Member Application


            The undersigned, the parent(s) or guardian of _______________________, hereby give our (my) consent and or permission for our (my) son/daughter/ward to join the Carrsville Volunteer Junior Fire Department. It is fully understood that he/she will not under any circumstances enter a burning structure, that he/she will only offer his/her assistance in bringing equipment to a firefighter of the senior department that is not a threat to his/her safety, and other activities at small fires and around the firehouse.
            It is further understood and agreed that he/she will have insurance coverage under the Carrsville Volunteer Fire Department policy to the limits of the said policy, and we (I) hereby relieve and save harmless the Carrsville Volunteer Fire Department from any and all liability for claim or claims in excess of the policy limit, or for any excess that the insurance does not pay for any lose.
